Step 3: Participants must type their first name, middle initial, and last name in the grey box. Then, select "Submit Survey."

Step 4: Once submitted, your attestation is complete, but you are not done! Go to the next module to complete the final exam. You must pass this final exam in order to receive credit for completing the class. This button will be an arrow if you are on a mobile device or will say "Next Module" if you are on a computer.
